Remaining silent without something nice to say gives agreeable speech priority. The saying cautions against needless unkindness.

What the words are saying

A qualified reading distinguishes gratuitous cruelty from necessary criticism, honest limits and reporting harm. Nice is not a sufficient test of whether information should be shared. A difficult truth can be conveyed considerately, and silence should not be required merely because another person finds a concern uncomfortable.
